Weight Loss Surgery

Digestive Surgery

Digestive surgery concerns procedures performed on the digestive system targeting the esophagus, stomach, duodenum, small intestine, colon and rectum

One of the indications for the management of bariatric surgery is a BMI ≥ 40
or a BMI ≥ 35 with at least one associated disease that may be improved
after surgery.
A normal BMI is between 18.5 and 25.
Calculation of BMI: m (kg) / h² (meter)

= body mass index
= weight (in kilograms)
= size (in meters)

Only a doctor can assess a patient’s eligibility for bariatric surgery and
decide on the most favorable technique.
Before considering this surgery, the patient should have a complete workup for
assess their current state of health and confirm that there are no contraindications
medical (physical and psychological) to the intervention.

Transluminal dilatation, percutaneous transluminal coronary angioplasty (PTCA) or percutaneous coronary intervention are similar which is a treatment which allows one or more coronary arteries to dilate which are narrowed or blocked by an atherosclerotic plaque which causes infarctions of the heart with or without cardiac arrest.

The procedure is performed under local anesthesia by an expert cardiologist with special attention during this procedure.

The stages of coronary angioplasty surgery :

1 – A coronary angiography is an X-ray examination using X-rays with a contrast product that allows you to view blocked arteries on a screen.

2 – A balloon is introduced through the femoral (above the leg) or radial (wrist) artery.

3 – The cardiologist lifts the balloon up to the targeted artery during the coronary angiography.

4 – The balloon inflates, which crushes the atherosclerotic plaque and widens the diameter of the artery.

5 – Blood circulation is restored with a new X-ray control which makes it possible to ensure that the objective is reached by the fact that the artery is perfectly unblocked.

6 – Placement of a simple or pharmacoactive stent (small spring) (covered with an antiproliferative substance) in the artery during the procedure to prevent it from being blocked again.

7 – End of the operation with return to the hospital room and prescription of antiplatelet therapy.

Average length of stay in France : 3-7 days.

Number of days of hospitalization : 1-2 days.

Gastric bypass is a so-called mixed technique since it consists of reducing gastric capacity (restriction of the stomach) and modifying the food circuit by reducing the caloric absorption of food in the small intestine (malabsorption).

DURING THE OPERATION:
The procedure is performed under general anesthesia and lasts between 1h30 and 3 hours.
This surgical technique consists of:
Remove part of the stomach to reduce its size to a small gastric pouch. This pocket thus limits the amount of food consumed.
Modifying the food circuit: part of the intestine is bypassed. Food no longer goes to the upper part of the small intestine but directly to its middle part. This results in decreased absorption of nutrients.
There are 2 types of bypass which differ in their production technique: the Roux en y bypass and the omega bypass.
AFTER THE OPERATION
The patient will stay in the recovery room for two hours then will generally be integrated into an intensive care unit.
Average length of stay in France 10 -14 days
Number of days of hospitalization 4 – 8 days

The goal of bariatric surgery is to lose weight in order to prevent and decrease associated diseases and improve the lives of patients in the long term.
However, all treatments and their results are patient-specific and will be part of the consultation with the doctor.
The doctor will specify the benefits, indications, precautions, clinical results, side effects and other important medical information specific to each technique of bariatric surgery.

This is an operation that retains a place in the therapeutic arsenal that can be offered to a patient suffering from severe, complicated or morbid obesity, in particular in cases of very young age or of high operative risk. AGA is a technique which decreases the volume of the stomach (restrictive) and does not interfere with the digestion of food. The diameter of the ring is changeable, it is placed around the upper part of the stomach, thus delimiting a small pocket in the stomach. Given the small size of the gastric pouch, the feeling of fullness appears quickly.

The expected effectiveness is 40-60% excess weight loss. Excess weight is a theoretical calculation of excess weight, it serves only as an evaluation of the effectiveness of the procedure. The calculation method is: example of a patient measuring 1m65, the “theoretical ideal” weight is 65 kilos. If his weight is 110 kilograms, the excess weight is 110-65 = 45 kilograms.
Being overweight is not the goal for the patient as the weight lost after this procedure may be more or less than the expected effectiveness.
Average length of stay in France 5 days
Number of days of hospitalization 1 day

Liver transplant (liver)

Definition:
Liver transplantation, or liver transplant, is a surgical procedure that involves replacing a diseased liver with a healthy liver, taken from a donor.
Indications

Decompensated cirrhosis,
Hepatocellular carcinoma
Severe acute liver failure.

Process:
All treatment options are discussed within the framework of a multidisciplinary staff involving all the specialists concerned before starting a registration on the national waiting list, the team will study one of the indications mentioned above. Once validated by the medical team, the registration process is carried out and the patient is assigned a score that will allow him or her to have more or less priority depending on the evolution of biological and clinical parameters.

Once a compatible brain-dead donor is found and accepted by the surgical team, the liver is harvested before being transplanted within hours of the donation between 6 a.m. and 8 a.m. after liver implantation.
